Gila County residents can now access the Internet to complete their jury questionnaire. You will need Internet access and your juror I.D. number which is located on the Jury Questionnaire above the bar   code and also above your name and address.

Clicking on the link button below will take you  directly to the page where you will be able to fill out the questionnaire on-line, saving you, the taxpayer time and postage.

If you choose the Internet Option, it is not necessary to mail the questionnaire back.

Pursuant to A.R.S § 21-202, documentation must be submitted for review on all requests for excusal.

Jury Duty Medical Form

Pursuant to HB2305 (ARS §21-202 B1 (b), if a patient requests that they be excused from jury service for reasons related to mental or physical conditions, Arizona law requires a written statement from a physician licensed by the state of Arizona. You can obtain a copy of the Jury Duty Medical Excuse form, which is to be used by all physicians excusing patients. To retrieve this document and print it out, click on the button below:

Juror Compensation

Those who are not selected as jurors but have appeared for the selection process will receive round trip mileage reimbursement at the state rate. Jurors who are sworn to serve on a trial will receive an additional $12 per day. If the trial lasts more than five days, additional compensation will be given.

One Day or One Trial

Gila County has implemented the "one day or one trial" practice which is mandated for the entire state effective January 1, 2005. Once you appear in response to a summons for jury duty, you will be excused until your name is randomly selected again for future service. If you are selected to serve as a trial juror, you will be excused for a period of two years.
