Small Claims Form.
This form is a complaint/summons form. The form can be printed for submission to
some local courts; however, please contact the court in which you will be
filing to confirm the use of this form as well as to verify the filing fees.
OTHER
Appearance Bond, also known as a bail bond, allows a defendant who is in jail to be released pending further court appearances. The purpose of the appearance bond is to guarantee the arrested person’s appearance at a time and place specified by the court. The amount of the bail bond varies with the seriousness of the charge and is set by the court.

Covenant Marriage in Arizona.
This booklet describes what steps must be taken to enter into a covenant marriage. It also lists the limited reasons available for a legal separation or divorce for those in a covenant marriage. Specific questions about covenant marriages should be directed to a member of the clergy, a marriage counselor or a legal advisor.

Finding an Attorney.
The State Bar of Arizona has several resources listed to help individuals locate an attorney in the state of Arizona.
Name Change. The Arizona Judicial Branch has provided several packets of forms and single forms in
PDF (Portable Document Format), which can be filled out online using a standard Internet Explorer 4.x or Netscape 4.x browser. The completed or blank forms can then be printed for submission to the court.
Self Service Center is designed to provide information and forms for persons who are representing themselves in court. The information contained on this
web site is intended to help you help yourself through the court system.
